Clonal evolution in maxillary sinus carcinoma

ScienceDaily (Sep. 28, 2012) ? Knowing how tumors evolve can lead to new treatments that could help prevent cancer from recurring, according to a study published September 29 by the Translational Genomics Research Institute (TGen) and Scottsdale Healthcare.

TGen researchers tracked several years of tumor evolution in a 47-year-old male patient with maxillary sinus carcinoma (MSC), a rare cancer of the sinus cavities beneath the cheeks that often requires surgical removal that is disfiguring. Fewer than half of MSC patients live more than 5 years after diagnosis.

"The ability to characterize clonal evolution of this rare cancer and identify its Achilles' heel can significantly impact treatment, leading to more personalized medicine," according to the study published in the journal PLOS ONE.

Clonal evolution refers to the often-rapid genetic changes that occur in cancer cells, which continually mutate and, thus, frequently resist anti-cancer drug compounds intended to destroy them.

"If we can understand the genomic basis of how this cancer evolves, perhaps we can find new treatments that could help improve the longevity and quality of life for patients," said Dr. Glen Weiss, Clinical Associate Professor at TGen, and Director of Thoracic Oncology at Virginia G. Piper Cancer Center Clinical Trials at Scottsdale Healthcare, a partnership with TGen. Dr. Weiss is one of the study's senior co-authors.

MSC represents nearly four of every five cases of paranasal sinus cancers, which grow rapidly and invade nearby tissues but also are usually slow to spread to distant sites. Patients usually die from a local recurrence of the tumor, even after aggressive treatment.

"This is the first report to study the clonal population of MSC arising in longitudinal samples from the same patient," the study said. "One of the aims of this study was to closely follow disease progression and the clonally evolving metastases for molecular profiling and accumulation of data for future use in development of personalized treatment."

The patient in the study received conventional treatment, which included surgical removal of his tumors, radiation therapy and chemotherapy, and participation in a clinical trial.

Over time, however, the cancer spread to his upper right lung, lower left lung, left kidney, brain and part of his intestine. He eventually was hospitalized, received hospice care and prior to passing away gave permission to have his cancer studied after death in a rapid autopsy research program.

"Because his cancer resumed growth despite several courses of systemic chemotherapy and radiation therapy, we speculated that acquired secondary genetic changes evolved with the evolution of resistance to these therapies," said Dr. Michael Barrett, Associate Professor in TGen's Clinical Translational Research Division, and the study's other senior co-author.

Analysis of his tumors following surgeries, biopsies and autopsy revealed several genetic aberrations, including multiple copies of a region on chromosome 4q, which includes the KIT gene. KIT is an oncogene, a gene with the potential to cause cancer, and is a potential treatment target.

The authors suggest the results provide a unique description of how the drug resistant cancer cells replicate and progress to metastatic MSC. Additional findings included the loss of the gene PKP4, which is associated with increased tumor size.

"These results show that molecular analyses of patient samples can add to the information about the tumor and help us in tracking back the progression of the disease," the authors concluded. "Identification of selected genetic changes, and the biological processes they regulate arising in primary MSC tumors, will advance individualizing therapy and improve the outcome of patients with rare cancers."

"These kinds of cutting-edge studies are made possible through the collaboration of major research and clinical practices, such as the partnership between TGen and Scottsdale Healthcare," said Dr. Mark Slater, Vice President of Research at Scottsdale Healthcare.

The authors remain particularly grateful to the patient and his family for their contribution to understanding more about this type of cancer and hope this dissemination of knowledge may help others.

New Melanoma Treatment Might Delay Cancer Progression - Drugs ...

SATURDAY Sept. 29, 2012 -- Researchers say they've discovered a two-drug combination that delays treatment resistance in patients with advanced melanoma.

By targeting different points in the same growth-factor pathway, the kinase inhibitor drugs dabrafenib and trametinib postponed the development of drug resistance in patients with BRAF-positive metastatic melanoma, the study authors said.

Melanoma is the most serious, and often deadly, form of skin cancer. In about half of patients with melanoma that has spread, tumor growth is caused by genetic mutations that keep the BRAF protein -- part of the MAPK cell growth pathway -- constantly activated. Drugs that inhibit BRAF activity can rapidly stop and reverse tumor growth in about 90 percent of patients. But the response is temporary in most cases, and tumor growth resumes in six or seven months, the researchers explained.

Previous research suggested that this drug resistance develops because the MAPK pathway gets turned back on through activation of MEK, another protein that is part off the MAPK pathway.

"We investigated this (drug) combination because of research we and others have conducted into the molecular underpinnings of resistance to BRAF inhibitor therapy," study lead author Dr. Keith Flaherty, of the Massachusetts General Hospital Cancer Center, said in a hospital news release.

The phase 1 and 2 study was sponsored by GlaxoSmithKline, which developed both drugs.

"We found that adding the MEK inhibitor trametinib to BRAF inhibitor dabrafenib clearly delays the emergence of resistance. In fact, the combination was at least twice as effective as BRAF inhibition alone," he said.

One expert agreed that the results were encouraging.

"This study addresses the problem of resistance to the mitogen-activated protein kinase [MAPK] pathway. This study combined dabrafenib [a selective BRAF inhibitor] and trametinib, [a selective MAPK kinase (MEK) inhibitor]. These drugs can be safely combined, and the survival was significantly improved without untoward side effects," said Dr. Michele Green, a dermatologist at Lenox Hill Hospital in New York City.

The study was conducted at 14 sites in the United States and Australia and included 162 patients who received different dose combinations of the drugs: two daily 150 milligram (mg) doses of dabrafenib plus one 2 mg dose of trametinib; the same dabrafenib dose with a 1 mg dose of trametinib; or treatment with dabrafenib alone.

Patients who received dabrafenib alone were able to receive the full-dose combination treatment if their cancer resumed progression.

Treatment with both combinations of dabrafenib and trametinib led to a four-month longer delay in drug resistance than treatment with dabrafenib alone. After one year of treatment, 41 percent of patients receiving the full-dose combination treatment had no progression of their melanoma, compared with 9 percent of those receiving dabrafenib alone.

Among patients receiving the combination treatment, the development of side effects such as skin rash and a less dangerous type of skin cancer called squamous cell carcinoma was similar to that typically seen in patients taking only one of the drugs.

The study was scheduled for presentation Saturday at the European Society for Medical Oncology meeting in Vienna and simultaneous publication in the New England Journal of Medicine.

The drug combination is now being tested in a larger phase 3 study, which is required for U.S. Food and Drug Administration approval.

DISH to Launch Satellite Internet - Zacks.com

DISH Network Corp.(DISH - Analyst Report) ? the second largest satellite TV operator in the U.S., has announced to launch satellite-based broadband service in the rural U.S. market starting from October, this year. DISH Network will become the first satellite TV provider in the U.S. to provide satellite net services in the rural areas as arch rival DIRECTV (DTV - Analyst Report) plans to launch similar services in 2013.

According to Federal Communication Commission (FCC) 14.5 million rural people doesn?t have access to high speed internet. The Englewood, Colorado-based company is trying to capture 2 million new customers with these services.

We believe, with the launch of this service DISH network will become a formidable challenger to large cable pay-TV and telecom service providers, which also offers triple-play video, voice, and data services. Furthermore, the launch of this service will allow it to diversify as the U.S. satellite market is already saturated.

Additionally, DISH Network is currently waiting for the FCC nod to launch a nationwide high-speed wireless broadband network to offer mobile Internet and phone services to its customers. Over 12 years of follow-up, those with lower levels of leptin were about four times more likely to develop Alzheimer?s than those with the highest levels.

The treatment with leptin has been shown in studies on animals recent to improve the performance of the memory, even after the onset of Alzheimer?s disease, such as dementia.

Discovered in the mid-1990, leptin is produced by fat cells and is considered critical for the regulation and. But there is evidence suggesting a role more and more of the hormone in brain development and memory.

Higher concentrations at the beginning of leptin were associated with lower rates of dementia years later. This association was observed even after adjusting for the impact of quarantine abdominal or belly fat, which was recently identified as a risk factor for Alzheimer?s disease early.

The study initially included 785 elderly participants in the Framingham Heart Study in progress, which began enrolling patients in 1948.

The National Institutes of Health recently awarded nearly $ 3 million for a small pilot study of leptin replacement therapy in patients with Alzheimer?s disease.

Pakistan Taliban: 'Amnesty' for bounty minister

ISLAMABAD (AP) ? The Pakistani Taliban announced Wednesday that they were giving a Cabinet minister an "amnesty," taking him off their hit list, because he offered a $100,000 bounty for the killing of an anti-Islam filmmaker.

Separately, a breakaway faction of the Afghan Taliban announced its own bounty for those behind "Innocence of Muslims," the film that has sparked deadly protests across the Islamic world. The group is offering almost $500,000 in gold.

Muslims have been angered by the crude, amateurish film's portrayal of the Prophet Muhammad as a fraud, a womanizer and a child molester. Dozens of people, including the U.S. ambassador to Libya, have died in violence linked to protests over the movie.

The main figure behind the video, Nakoula Basseley Nakoula, has put his home up for sale and gone into hiding since violence erupted over the 14-minute YouTube trailer for "Innocence of Muslims," which also has sparked debate over freedom of expression in the U.S. and in Europe.

Some of the most intense protests have erupted in Pakistan, where the role of Islam in society is sacrosanct and anti-American sentiment runs high. It was in that atmosphere that Railways Minister Ghulam Ahmad Bilour offered his $100,000 reward Saturday for anyone who kills Nakoula.

Bilour also appealed to al-Qaida and Taliban militants to help eliminate the filmmaker, while Pakistan's government said the bounty reflected Bilour's personal view and was not official policy.

The minister belongs to the secular Awami National Party, an ally in the government of President Asif Ali Zardari. His comments drew criticism within his own party, which is considered anti-Taliban and has lost several leaders in the fight against the insurgency.

Pakistani Taliban spokesman Ahsanullah Ahsan told The Associated Press by telephone Wednesday that the minister's views represent the true spirit of Islam, and that, consequently, the insurgents have removed him from their hit list.

But while Bilour gets an "amnesty," others in his party are still fair game, Ahsan said. Bilour could not immediately be reached for comment.

In Afghanistan, the Taliban faction known as the Dadullah Group offered eight kilograms, or 17.6 pounds, of gold to anyone who kills the film's creators. In a telephone message to the AP, spokesman Rehbar Mal, announced the reward, which would be the equivalent of about $487,000.

The group, named for a Taliban commander killed by NATO troops in 2007, broke away from the Afghan Taliban earlier this year to protest reconciliation talks with the United States. The Afghan Taliban and the Pakistani Taliban are separate, though linked, militant outfits.

Fix the Machine, Not the PersonWhen people mess up, we often react by getting angry and yelling at them. We want to change the way they're acting. As Aaron Swartz illustrates, responding with anger just makes people more defiant. The key is to fix the situation, not the person?whether that means changing how you behave, bringing another person into the mix, or revamping entire plans.

The GM Experiment

The General Motors plant in Fremont was a disaster. "Everything was a fight," the head of the union admits. "They spent more time on grievances and on things like that than they did on producing cars. They had strikes all the time. It was just chaos constantly. ?It was considered the worst workforce in the automobile industry in the United States."

"One of the expressions was, you can buy anything you want in the GM plant in Fremont," adds Jeffrey Liker, a professor who studied the plant. "If you want sex, if you want drugs, if you want alcohol, it's there. During breaks, during lunch time, if you want to gamble illegally-any illegal activity was available for the asking within that plant." Absenteeism was so bad that some mornings they didn't have enough employees to start the assembly line; they had to go across the street and drag people out of the bar.

When management tried to punish workers, workers tried to punish them right back: scratching cars, loosening parts in hard-to-reach places, filing union grievances, sometimes even building cars unsafely. It was war.

In 1982, GM finally closed the plant. But the very next year, when Toyota was planning to start its first plant in the US, it decided to partner with GM to reopen it, hiring back the same old disastrous workers into the very same jobs. And so began the most fascinating experiment in management history.

Toyota flew this rowdy crew to Japan, to see an entirely different way of working: The Toyota Way. At Toyota, labor and management considered themselves on the same team; when workers got stuck, managers didn't yell at them, but asked how they could help and solicited suggestions. It was a revelation. "You had union workers-grizzled old folks that had worked on the plant floor for 30 years, and they were hugging their Japanese counterparts, just absolutely in tears," recalls their Toyota trainer. "And it might sound flowery to say 25 years later, but they had had such a powerful emotional experience of learning a new way of working, a way that people could actually work together collaboratively?as a team."

Three months after they got back to the US and reopened the plant, everything had changed. Grievances and absenteeism fell away and workers started saying they actually enjoyed coming to work. The Fremont factory, once one of the worst in the US, had skyrocketed to become the best. The cars they made got near-perfect quality ratings. And the cost to make them had plummeted. It wasn't the workers who were the problem; it was the system.1

An organization is not just a pile of people, it's also a set of structures. It's almost like a machine made of men and women. Think of an assembly line. If you just took a bunch of people and threw them in a warehouse with a bunch of car parts and a manual, it'd probably be a disaster. Instead, a careful structure has been built: car parts roll down on a conveyor belt, each worker does one step of the process, everything is carefully designed and routinized. Order out of chaos.

Fixing the Machine

And when the system isn't working, it doesn't make sense to just yell at the people in it?any more than you'd try to fix a machine by yelling at the gears. True, sometimes you have the wrong gears and need to replace them, but more often you're just using them in the wrong way. When there's a problem, you shouldn't get angry with the gears?you should fix the machine.

If you have goals in life, you're probably going to need some sort of organization. Even if it's an organization of just you, it's still helpful to think of it as a kind of machine. You don't need to do every part of the process yourself?you just need to set up the machine so that the right outcomes happen.

For example, let's say you want to build a treehouse in the backyard. You're great at sawing and hammering, but architecture is not your forte. You build and build, but the treehouses keep falling down. Sure, you can try to get better at architecture, develop a better design, but you can also step back, look at the machine as a whole, and decide to fire yourself as the architect. Instead, you find a friend who loves that sort of thing to design the treehouse for you and you stick to actually building it. After all, your goal was to build a treehouse whose design you like?does it really matter whether you're the one who actually designed it?2

Or let's say you really want to get in shape, but never remember to exercise. You can keep beating yourself up for your forgetfulness, or you can put a system in place. Maybe you have your roommate check to see that you exercise before you leave your house in the morning or you set a regular time to consistently go to the gym together. Life isn't a high school exam; you don't have to solve your problems on your own.

Fundamental Attribution Error

In 1967, Edward Jones and Victor Harris gathered a group of college students and asked them to judge another student's exam (the student was a fictional character, but let's call him Jim). The exam always had one question, asking Jim to write an essay on Fidel Castro "as if [he] were giving the opening statement in a debate." But what sort of essay Jim was supposed to write varied: some of them required Jim to write a defense of Castro, others required Jim to write a critique of Castro, the rest left the choice up to Jim. The kids in the experiment were asked to read Jim's essay and then were asked whether they thought Jim himself was pro- or anti-Castro.

Jones and Harris weren't expecting any shocking results here; their goal was just to show the obvious: that people would conclude Jim was pro-Castro when he voluntarily chose write to a pro-Castro essay, but not when he was forced to by the teacher. But what they found surprised them: even when the students could easily see the question required Jim to write a pro-Castro essay, they still rated Jim as significantly more pro-Castro. It seemed hard to believe. "Perhaps some of the subjects were inattentive and did not clearly understand the context," they suspected.

So they tried again. This time they explained the essay was written for a debate tournament, where the student had been randomly assigned to either the for or against side of the debate. They wrote it in big letters on the blackboard, just to make this perfectly clear. But again they got the same results?even more clearly this time. They still couldn't believe it. Maybe, they figured, students thought Jim's arguments were so compelling he must really believe them to be able to come up with them.

So they tried a third time?this time recording Jim on tape along with the experimenter giving him the arguments to use. Surely no one would think Jim came up with them on his own now. Again, the same striking results: students were persuaded Jim believed the arguments he said, even when they knew he had no choice in making them.3

This was an extreme case, but we make the same mistake all the time. We see a sloppily-parked car and we think "what a terrible driver," not "he must have been in a real hurry." Someone keeps bumping into you at a concert and you think "what a jerk," not "poor guy, people must keep bumping into him." A policeman beats up a protestor and we think "what an awful person," not "what terrible training." The mistake is so common that in 1977 Lee Ross decided to name it the "fundamental attribution error": we attribute people's behavior to their personality, not their situation.4

Changing the Situation

Our natural reaction when someone screws up is to get mad at them. This is what happened at the old GM plant: workers would make a mistake and management would yell and scream. If asked to explain the yelling, they'd probably say that since people don't like getting yelled at, it'd teach them be more careful next time.

But this explanation doesn't really add up. Do you think the workers liked screwing up? Do you think they enjoyed making crappy cars? Well, we don't have to speculate: we know the very same workers, when given the chance to do good work, took pride in it and started actually enjoying their jobs.

They're just like you, when you're trying to exercise but failing. Would it have helped to have your friend just yell and scream at you for being such a lazy loser? Probably not?it probably would have just made you feel worse. What worked wasn't yelling, but changing the system around you so that it was easier to do what you already wanted to do.

The same is true for other people. Chances are, they don't want to annoy you, they don't like screwing up. So what's going to work isn't yelling at them, but figuring out how to change the situation. Sometimes that means changing how you behave. Sometimes that means bringing another person into the mix. And sometimes it just means simple stuff, like changing the way things are laid out or putting up reminders.

At the old GM plant, in Fremont, workers were constantly screwing things up: "cars with engines put in backwards, cars without steering wheels or brakes. Some were so messed up they wouldn't start, and had to be towed off the line." Management would yell at the workers, but what could you do? Things were moving so fast. "A car a minute don't seem like it's moving that fast," noted one worker, "but when you don't get it, you're in the hole. There's nobody to pull you out at General Motors, so you're going to let something go."

At the Toyota plant, they didn't just let things go. There was a red cord running above the assembly line, known as an andon cord, and if you ever found yourself in the hole, all you had to do was pull it, and the whole line would stop. Management would come over and ask you how they could help, if there was a way they could fix the problem. And they'd actually listen?and do it!

You saw the results all over the factory: mats and cushions for the workers to kneel on; hanging shelves traveling along with the cars, carrying parts; special tools invented specifically to solve problems the workers had identified. Those little things added up to make a big difference.

When you're upset with someone, all you want to do is change the way they're acting. But you can't control what's inside a person's head. Yelling at them isn't going to make them come around, it's just going to make them more defiant, like the GM workers who keyed the cars they made.

No, you can't force other people to change. You can, however, change just about everything else. And usually, that's enough.

(AP) ? Two suicide car bombers struck Syria's army command headquarters Wednesday in Damascus, killing four guards and engulfing a key symbol of President Bashar Assad's embattled regime in flames, state media and witnesses said.

The twin blasts were followed by several hours of gunbattles between rebel fighters and regime forces in downtown Damascus. A reporter for an Iranian TV channel was killed by gunfire near the clashes, and a correspondent for another Iranian station was wounded.

The brazen rebel attacks in the heart of the Syrian capital highlighted their determination to bring down Assad as the country's civil war intensifies. It was the second day of bombings to shake the capital.

A Damascus school that activists said was being used by regime forces as a security headquarters also was bombed on Tuesday, wounding several people.

Near Damascus, two activist groups monitoring casualties said several dozen bodies were found Wednesday in a single location.

Syria's unrest began in March 2011 when protests calling for political change met a violent government crackdown. Many in the opposition have since taken up arms, and activists say the fighting has killed nearly 30,000 people.

Over the past few months, the rebels have increasingly targeted security sites and symbols of regime power in a bid to turn the tide in the fighting.

The rebels' Free Syrian Army claimed responsibility for Wednesday's bombings.

The blasts went off about 10 minutes apart, starting around 7 a.m., near the landmark Omayyad Square, and were heard several miles (kilometers) away.

Syrian state TV aired what it said was security camera footage of the blasts. In the first, a white van is driving on the road outside the military headquarters, then veers to the right and explodes. The second blast goes off inside the compound, with flames rising from behind trees.

Later, the army command building is seen engulfed in flames that sent huge columns of thick black smoke over Damascus for several hours.

The explosions shattered the windows of the Dama Rose hotel and other nearby buildings, as well as windshields of parked cars. Footage by another state-run TV channel, Ikhbariya, showed heavy damage inside the compound, with glass shards scattered across the floor and broken ceiling tiles.

The blasts caused fear among residents of a nearby upscale district, which has largely been sheltered from the violence that plagues other parts of the city.

"What if a random bullet killed one of my kids?" Nada, a 42-year-old mother of three who only gave her first name out of security concerns, said, crying over the telephone. The windows of her apartment were shattered and her furniture was damaged. "I only care about my children and I'm afraid of the gunfire," she added.

Gaith, 63, a retired civil servant who also just gave one name for the same reason, said he rushed to lock the gate of his building to keep rebels from hiding inside. "I don't want my place to collapse on my head," he said.

A Syrian army statement said "terrorists" ? a term the regime routinely uses for the rebels ? in the area simultaneously opened fire randomly to scare people, adding that authorities were pursuing the gunmen.

Witnesses said the explosions were followed by heavy gunfire that stretched on for hours near Omayyad Square and around the military compound. One witness reported seeing panicked soldiers shooting in the air randomly as they ran.

A group of army soldiers standing outside the buildings shouted pro Assad slogans, including: "Shabiha, forever, for your eyes, Oh Assad!" in reference to pro-regime militiamen.

Information Minister Omran Zoubi played down the rebels' ability to strike the nerve center of the Assad regime's battle for survival.

"Everything is normal," he told Syrian TV, which is near the army command center, in a phone call. "There was a terrorist act, perhaps near a significant location, yes, this is true, but they failed as usual to achieve their goals."

Zoubi initially insisted there were no casualties. However, Syrian state TV later reported four army guards were killed and 14 people were wounded, including civilians and military personnel.

The Iranian English language Press TV said one of its correspondents, 33-year-old Maya Nasser, a Syrian national, died in an exchange of fire in the area following the blasts. Another journalist working for Iranian Al-Alam TV was wounded, according to the station.

Press TV said Nasser was shot and killed by a rebel sniper while reporting on air. The station's newsroom director Hamid Reza Emadi blamed Turkey, Saudi Arabia and Qatar because they "provide weapons and militants to kill civilians, military personnel and journalists."

Iran is a supporter of the Syrian regime, while Turkey, Saudi Arabia and Qatar back the rebels.

Damascus, the stronghold of the regime, has been targeted repeatedly by the rebels. Previous bombings raised concerns that the al-Qaida terror network is becoming increasingly active in Syria. Jebhat al-Nusra, an extremist Syrian group, claimed responsibility for many of them.

On July 18, rebels detonated a bomb inside a high-level crisis meeting in Damascus that killed four top regime officials, including Assad's brother-in-law and the defense minister. Other large blasts have targeted the headquarters of security agencies in the capital, killing scores of people this year.

In other developments Wednesday, two activist groups monitoring casualties said dozens of bodies were found in a southern Damascus suburb. The reports could not be independently confirmed because of strict restrictions on foreign media.

The Britain-based Syrian Observatory for Human Rights said 40 bodies, including some of women and children, were found in the suburb of Thiyabiyeh. Rami Abdul-Rahman, the head of the Observatory, said he did not have details about how the victims were killed.

Another group of activists, the Local Coordination Committees, said 107 bodies were found, including women and children killed execution-style. It said the dead included nine members of the Al-Rifaie family whose throats were slit.

An amateur video, which could not be verified, showed the bodies of 18 men lined up on the floor of a room, some of them with marks of deep wounds.

Scientists hope gene breakthrough could help men with breast cancer

Scientists have found a gene which can increase the risk of male breast cancer by 50 per cent, after the world's largest study into the disease.

Screening of the genetic codes of more than 800 male breast cancer patients, mainly from the UK, has revealed that mutations in one particular gene play a role in the disease, and could herald the development of new treatments specifically for men.

Very little is known about the causes of male breast cancer, which is diagnosed in 350 men in the UK every year, compared with 48,000 women. Previously researchers uncovered that faulty BRCA2 genes are involved in around 10 per cent of male breast cancer cases but they have now established that faulty RAD51B genes, which have also been found to increase the risk of breast cancer in women, also play a role in the disease in men.

Dr Nick Orr, a group leader at the Breakthrough Breast Cancer Research Centre at The Institute of Cancer Research, London, and one of the study's authors, said: "This study represents a leap forward in our understanding of male breast cancer."

The research published today in Nature Genetics will help doctors predict whether men are more at risk of developing breast cancer. " For a general man in the population this latest news isn't that significant because the risk of breast cancer in males is already so low," Dr Orr said. "But for people who are already at high risk of breast cancer, for example men who have mutations in the BRCA2 gene, having this extra information could really help our ability to predict whether they are going to get the disease or not."

Scientists from the Breakthrough Breast Cancer Research Centre were part of an international research team involved in the Genome-Wide Association Study (GWAS), which investigated 447,000 alterations in the DNA of participants as part of a wider four-year-long study. Scientists now hope to build on the success of the GWAS' findings to identify further causes of both male and female breast cancer.

Case study

Andrew Tokely, 47, is a horticulturalist from Capel St Mary, Suffolk. He is now in remission after being diagnosed with breast cancer in 2009.

When I was diagnosed with breast cancer I didn't even know men could get the disease. A lot of people didn't know what to say, so they didn't say anything. But most people were shocked because, like me, you just don't hear of men with breast cancer. One charity calls it 'the women's fight,' and very few mention that it can happen to men. There's stupid things too, like the medication we're put on afterwards, tamoxifen. Nothing on that leaflet or packet gives you a side-effect for a man.

But you get them.

Japan's Noda to remain PM after ruling party vote

TOKYO (AP) ? Prime Minister Yoshihiko Noda handily beat three other contenders in a ruling party leadership election Friday and will remain Japan's leader a while longer despite growing voter disappointment with his government's performance.

Noda, in office a year, won 818 points out of a total of 1,231 points in the vote, suggesting the ruling Democratic Party of Japan has rallied around him even as his approval rating has fallen below 30 percent.

Noda had said he plans to call a national election "before long," but has given no timeframe. Lately, he has suggested he intends to stay on in office to try to finish tasks he set out to accomplish, including helping Japan deal with the impact of last year's tsunami and nuclear crisis.

Polls suggest the DPJ would be badly defeated if elections were held now.

Voters appear to be disappointed over the DPJ's inability to deliver promised change to Japan's stodgy politics and are upset with Noda's push to double the sales tax to 10 percent, a step Noda argues is needed to meet increasing social security costs as Japan's population ages and its national debt grows.

Noda, Japan's sixth prime minister in six years, played up his resolve to make tough decisions in a speech before Friday's vote, promising to "sweat with all of you to make a vigorous Japan together."

"The real reform Japan needs is decisive politics when we face issues that need to be decided," Noda told party members gathered in a Tokyo hotel.

Noda also pledged to push forward with the goal of making Japan a nuclear free society by the 2030s. The pledge appeared to be an attempt to show his government was committed to phasing out nuclear power even though his Cabinet on Wednesday backpedaled from an advisory panel's recommendation to eliminate nuclear power by 2040 in the face of opposition from pro-nuclear businesses and groups.

In addition to issues at home, Noda's government has been pressured lately by a territorial dispute with China over uninhabited islands in the East China Sea controlled by Japan but also claimed by China and Taiwan.

Many analysts see the main opposition Liberal Democratic Party winning the next election ? which must be called by next September ? although falling short of a majority. Analysts believe elections will most likely be held early next year.

Noda struck a deal earlier this year to gain support from the LDP and another opposition party for the tax hike and social security reforms in exchange for a promise to call an early election. But Japanese media have reported Noda wants to put that deal on ice because the LDP backed passage of a censure motion against him in the opposition-controlled upper house.

In a bid to show he can deliver results, Noda may demand the LDP's support on more bills before calling elections, said Steven Reed, a political science professor at Chuo University in Tokyo. He noted that voters have grown frustrated over the gridlock and political infighting that has dominated parliament in recent years.

"They want someone who can get stuff done," Reed said. "They want an end to immobilism. They want leadership. They want a unified party. They want a party that isn't falling apart every time they turn around."

The LDP, a conservative, pro-business party with a nationalist bent, is holding its own leadership election on Wednesday. The winner could become the next prime minister if the LDP comes out on top in the next elections.

Front-runners among the five candidates include former Prime Minister Shinzo Abe, ex-defense chief Shigeru Ishiba and party secretary-general Nobuteru Ishihara, the son of Tokyo's outspoken governor, Shintaro Ishihara, who started the latest territorial furor with his plan to buy and develop the disputed islands also claimed by China.

Vying against Noda in the DPJ leadership race were two former farm ministers, Michihiko Kano and Hirotaka Akamatsu, and a former internal affairs minister, Kazuhiro Haraguchi.
